Mr Ashley Iain Simpson

Mr Ashley Ian Simpson is a dual‑fellowship‑trained Consultant Orthopaedic Surgeon whose subspecialty centres on adult and paediatric brachial‑plexus and peripheral‑nerve surgery together with comprehensive upper‑limb reconstruction. He holds a substantive post at the Royal National Orthopaedic Hospital, Stanmore, within the United Kingdom’s national Peripheral Nerve Injury Unit. In this role he routinely assesses, surgically manages, and provides medico‑legal evaluation for a wide spectrum of peripheral‑nerve pathologies, ranging from complex traumatic nerve injuries and nerve tumours to iatrogenic and compressive neuropathies, as well as post‑traumatic neuropathic pain and complex regional pain syndrome. His operative repertoire includes microsurgical nerve repair, grafting, functional muscle transfers, and sophisticated shoulder and elbow reconstructions, all aimed at restoring function and alleviating pain.

Prior to his current appointment, Mr Simpson served as Upper‑Limb Consultant and Orthopaedic Education Lead at Guy’s and St Thomas’ Hospital, London. He completed fellowships in Upper‑Limb Surgery in London and Sports & Shoulder Surgery in Australia, and has undertaken international visiting fellowships in Peripheral Nerve Surgery and Complex Upper‑Limb Reconstruction. In addition to his clinical duties, he prepares approximately ninety medicolegal reports each year, with a claimant : defendant : joint ratio of 50 : 40 : 10, and frequently provides expert testimony in clinical‑negligence and personal‑injury cases involving orthopaedic trauma and peripheral‑nerve injury.

Mr Simpson’s clinical interests span the entire continuum of peripheral‑nerve and upper‑limb pathology. He is deeply engaged in the management of brachial‑plexus injuries, both traumatic and obstetric, employing nerve transfers, grafts and functional muscle transfers to re‑establish motor function. His expertise in peripheral‑nerve injuries encompasses acute traumatic lacerations, chronic compressive lesions and iatrogenic damage, for which he utilises microsurgical repair, autologous grafting and neuro‑tisation techniques. Nerve tumours, including schwannomas, neurofibromas and malignant peripheral nerve sheath tumours, are approached with a focus on complete excision while preserving nerve continuity and function.

Reconstruction of the shoulder forms a further pillar of his practice; he addresses rotator‑cuff tears, instability, arthritic degeneration, complex fractures and performs arthroplasty when indicated. Thoracic outlet syndrome—whether neurogenic, venous, arterial or post‑traumatic—is managed through comprehensive decompression and, where necessary, vascular reconstruction. Complex upper‑limb trauma involving the clavicle, scapula, humerus, elbow, forearm, wrist and hand is treated with integrated bone, nerve and vascular repair strategies.

Entrapment neuropathies such as carpal‑tunnel, cubital‑tunnel, radial‑tunnel, suprascapular and axillary compressions are released surgically, complemented by ergonomic advice and structured rehabilitation programmes. Finally, Mr Simpson is actively involved in the multimodal management of post‑traumatic neuropathic pain and complex regional pain syndrome, integrating surgical decompression, neuromodulation, pharmacotherapy and specialised pain‑management pathways. Through this broad and evolving portfolio, he remains at the forefront of peripheral‑nerve surgery, continuously advancing both his operative techniques and his contributions to the medico‑legal field.

Mr Simpson’s research focuses on integrating technology with peripheral‑nerve care. He is developing artificial‑intelligence tools for nerve diagnosis and surgical planning, while also identifying imaging biomarkers that track injury and degeneration. He leads the creation of a national registry to capture long‑term outcomes after nerve trauma, and evaluates clinical results of nerve‑sheath tumour resections and thoracic‑outlet decompression for post‑traumatic plexopathy. To date he has authored over 50 peer‑reviewed papers, contributed textbook chapters and presented internationally on nerve injury, shoulder reconstruction and AI‑driven surgery.

Mr Simpson is an active educator, serving as faculty on the Miller FRCS Trauma & Orthopaedics Revision Course and directing both the London Brachial Plexus & Peripheral Nerve Course and the Physician Associates Surgical Skills & Assisting Course. He also functions as an examiner for the medical schools at University College London and King’s College London and participates as an interviewer in the UK national ST3 orthopaedic recruitment process.

Mr Simpson’s leadership portfolio extends well beyond the operating theatre, reflecting a broad commitment to service development, education and professional governance. He currently sits on the NHS England Specialised Services Commissioning Work Group for Peripheral Nerve Surgery, where he contributes to national policy and pathway design for these high‑complexity services. Within the British Elbow and Shoulder Society he serves as an Education Committee Member, shaping curricula and training standards for the next generation of elbow‑and‑shoulder specialists. His expertise is further recognised by his appointment to the Shoulder & Elbow Sub‑Committee of SICOT, where he participates in the formulation of international consensus statements and clinical guidelines. Mr Simpson also chairs the Clinical Advisory Board and acts as a Trustee for Pain UK, providing strategic oversight for the charity’s clinical programmes and research initiatives. In the academic sphere he is a faculty member of the King’s Health Partners Executive Fellowship in Surgical Leadership, mentoring senior surgeons in management and quality‑improvement techniques, and he regularly reviews manuscripts for the Injury journal and the Journal of Experimental Orthopaedics. As the Virtual Education Lead for the Global Nerve Foundation, he coordinates worldwide webinars and e‑learning modules that disseminate cutting‑edge knowledge in peripheral‑nerve surgery. Finally, he founded the London Brachial Plexus & Peripheral Nerve Course, an annual intensive training programme that brings together trainees, fellows and senior clinicians to refine surgical skills and discuss emerging research. Together, these roles demonstrate Mr Simpson’s extensive experience in clinical governance, education, research appraisal and strategic leadership within orthopaedics and peripheral‑nerve surgery.
