Perineal Trauma Expert Witnesses

Perineal trauma can occur during vaginal childbirth when the perineum tears or a clinician performs an episiotomy. Injuries range from skin and muscle tears to more severe injuries involving the anal sphincter. Third- and fourth-degree tears are also known as obstetric anal sphincter injuries (OASI). Cases involving childbirth-related perineal injuries can raise complex medico-legal questions. These may relate to risk assessment, delivery, diagnosis, classification, repair and follow-up care. Experts may also consider pain, continence problems, sexual function and long-term outcomes. RCOG recognises these as potential consequences of severe perineal injury. Experts may review maternity records, delivery documentation, operative records and follow-up assessments. They can also assess the nature and consequences of the injury.
